Asthma Action Plan
What is an asthma action plan?
A plan to provide information in writing on how to manage your specific asthma needs. The plan may include: Medications, recognizing triggers (except physical activity), tracking your level of asthma control, and what to do in an emergency. This plan is usually written during a discussion with your doctor and can be shared with teachers, school nurses, and other caregivers to ensure your asthma is properly treated. A plan may be downloaded here and taken to your healthcare provider to help you complete it. Your child should leave a copy with the school or daycare nurse.
